Payment is limited to the applicable percentage of the dentist's fees up to the Maximum Plan Allowance for a specific procedure. You are required to pay the balance of any such fee or allowance, known as a copayment. It is to your advantage to select a dentist who is a Delta Dental dentist since a lower percentage of the dentist's fees may be covered by the plan if you select a Non-Participating dentist.

**No deductible required for diagnostic or preventive services.

Payment for any single procedure that is a covered service will be made upon completion of the procedure. Payment for care is applied to the calendar year deductible and maximum benefit based on the date of service, regardless of when the claim is submitted. After you have satisfied your deductible requirement, Delta Dental will provide payment for covered services at the percentage indicated in the chart above up to the maximum benefit for your selected plan for each subscriber in each calendar year.
